You can find this application on the internet. After installing the application, open the PDF form using Ghostscript. When you open this PDF document, go to the menu and click on convert button. Then select Jpeg option in Device section. You will find this in Convert window. The resolution for opening the form should be 300. Then select all the PDF format pages, which are to be edited. In PowerPoint go to the Page Setup tab and edit the height and width of the document. Set the width to 8.5 and height to 11. Then select Slide Master tab in the View section. The Slide Mater window will help you to edit a PDF form by using PowerPoint.
